Long-term follow-up pathway
TYA looked after in the GOSH long- term follow- up clinic
- If you completed your treatment for cancer more than 5 years ago, you will be being seen in the GOSH long term follow-up clinic, usually once a year.
- When you turn 17, you will have a special transition appointment to prepare you for the transition across to the UCLH Long term follow up clinic.
- During this appointment you will be given information about the cancer you were treated for and what you might need to look out for in the future.
- The team at GOSH will give a thorough handover to the team at UCLH and your next appointment will be at UCLH.
TYA looked after in the UCLH TYA follow-up clinic
- If you have completed treatment and are being followed up at UCLH but are turning 18, you will move to the adult long term follow up clinic. You will have an appointment with one of the long- term follow up CNS’s who will prepare you for the move to the adult long term follow up clinic.
