Transition & Developmentally Appropriate Healthcare

The transition process can be overwhelming for many young people with complex health conditions. Developmentally appropriate healthcare allows bespoke treatment for young people with their changing developmental needs.

As one of the network’s cross-cutting themes, the aim is to ensure that this process is as smooth as possible for patients and professionals. Transition is vital for all specialties and each has their own barriers when carrying out the transition process. This workstream is dedicated to breaking down these barriers for professionals. We are currently collating information from all NTPN services on their transition processes in order to be able to provide specific education and advice.
