Nephrology

The paediatric nephrology service at Great Ormond Street Hospital serves a paediatric population of around 2.5 million children across North Thames and surrounding regions. We offer inpatient and outpatient facilities through 14 inpatient nephrology beds, 17 haemodialysis units and outpatient facilities with around 7000 outpatient reviews at GOSH annually. Additional outreach clinics with paediatric colleagues at Lister Hospital, Royal London Hospital and Colchester Hospitals. We offer telephone advice 24/7 to hospitals within our network.

Our services include tertiary paediatric nephrology services for dialysis (in-centre and home therapies), kidney transplantation, chronic kidney disease, nephrotic, glomerular, tubular, stone, renal genetics, hypertension, renovascular and general nephrology and we receive quaternary referrals for complex kidney transplantation and for renovascular endovascular and surgical interventions.

Our department includes 12 consultants with Dr Matko Marlais as our current speciality lead, there are 7 consultant paediatric kidney transplant surgeons, 5 Advanced Nurse Practitioners, 8 Clinical Nurse specialists and one Physician Associate.

Dialysis

At Great Ormond Street Hospital, we provide a comprehensive dialysis service for children offering:

The in-centre HD unit is open Monday to Saturday but does not offer an on-call service. The peritoneal dialysis and home haemodialysis services are both outpatient services and the team are available Monday to Friday during office hours.

The low-clearance service where children and young people are seen prior to initiation of kidney replacement therapy are reviewed in a weekly clinic. We accept referrals through our network clinicians and quaternary referrals from eGFR <20 mL/min/1.73m2.

The weekly glomerular clinic is run by three consultant paediatric nephrologists (Professor Stephen Marks, Drs Matko Marlais and Louise Oni) and one advanced nurse specialist, Ms Hazel Webb.  We provide holistic care for patients up to the age of 18 years with glomerulopathies and glomerulonephritis.  This includes diagnostic work-up, including percutaneous renal biopsies and genetics studies to find the cause of your child’s inflammatory kidney condition.  We look after patients with systemic lupus erythematosus, lupus nephritis, post-infectious glomerulonephritis, immune-mediated glomerulonephritis (including C3 glomerulopathies, glomerulonephritis and dense deposit disease) and vasculitis (including ANCA associated vasculitis) as well as other kidney conditions.  

We provide treatment plans of immunosuppression and other medications working with you, your child and your general paediatric team with shared care with local consultant paediatricians.  We will inform you of potential clinical trials for your child and their kidney discussion, working closely with our National Institute of Health and Care Research (NIHR) Great Ormond Street Hospital Clinical Research Facility (please see https://www.gosh.nhs.uk/our-research/nihr-gosh-clinical-research-facility).

Stones in the kidney system are uncommon in children. Our dedicated multidisciplinary team provides comprehensive care for children with urinary stones and associated metabolic conditions. We are one of the most experienced children’s stone units in Western Europe. We have expertise in management and prevention of stones, of varying complexity, in the urinary system for patients under 18 years of age.

Conditions we treat

  • Stones in the kidneys, ureters or bladder
  • Acutely obstructing stones in the urinary tract
  • Diagnosis & management of underlying causes that increase risk of stone recurrence
  • Metabolic conditions predisposing to stones including: Cystinuria
  • Suspected or confirmed Primary Hyperoxaluria
  • Urinary stones can affect children with other significant diagnoses which we are experienced in managing here at Great Ormond Street Hospital (GOSH)

Our service has:

  • Dedicated multi-disciplinary stones outpatient clinic to provide a combined nephrological (medical), urological (surgical) assessment.
  • Comprehensive metabolic and genetic diagnostic service
  • Dietician and specialist nursing support
  • Specialist Radiology including Ultrasound, ultra-low dose computer tomography and DMSA-SPECT imaging.
  • Modern OR-1 integrated operating theatre with state-of-the-art equipment including for keyhole stone surgery a range of ureteroscopes and laser for stone disintegration. Learn more about endoscopic removal of kidney stones
  • A mobile lithotripter service for Extra-corporeal Shock Wave Lithotripsy (ESWL).
  • Monthly multi-disciplinary meetings to optimise management of complex case.

The Nephrotic service at GOSH treats one of the largest cohorts of nephrotic patients in UK. With more than 300 nephrotic patients, it is run through weekly clinics delivered by Drs Zainab Arslan and Louise Oni and advanced nurse practitioner Hazel Webb. Service administrator is Ms Simonetta Fisogni. We accept referrals for complex nephrotic including children with steroid dependant nephrotic syndrome, frequently relapsing nephrotic syndrome and steroid resistant nephrotic syndrome through secondary care physicians. We share the care of all our patients with their local hospitals.

Renal transplant service at GOSH is the largest in the UK and second largest in Europe. We perform 30-35 transplants per year and currently oversee care of over 160 post-transplant patients. Service receives national and international quaternary referrals for children with complex surgical and medical conditions and has the largest cohort of paediatric antibody incompatible kidney transplant recipients in the UK. Transplant service has pioneered the use of 3D printed models to facilitate complex transplantation and improve consenting pathway. Excellent clinical outcomes are a result of multidisciplinary team which includes nine medical and surgical consultants, one advanced nurse practitioner, one clinical nurse specialist, physician associate, dieticians, psychologists, social worker and pharmacists.   It is research led and evidence-based service. Service has joint liver and kidney clinic for children with multi-solid organ transplants Current transplant lead is Dr Jelena Stojanovic. Other team members include Prof Stephen Marks, Dr Zainab Arslan, advanced nurse practitioner Sheila Boyle, CNS Diana Korit, Physician Associate Fred Tomlin and Service Administrator Ann Alexander.
